United Uniform's commitment to the Law Enforcement and Public Safety Communities necessitate that certain items only be sold to the proper personnel. Please note that there may be other restrictions based on state and local laws that prohibit the possession or use of certain products in your area. It is your duty to be aware of the local and state restrictions and cannot hold United Uniform responsible for the misuse or possession of any product.

The following form will need to be filled out for any restricted item. At any time during the order process items may show the ID Check Logo, meaning the item requires identification verification to ensure this restricted item is only sold to the appropriate personnel. Please fill out this form once the order has been completed, only complete the verification process after ordering. United Uniform reserves the right to contact supervisory personnel in order to confirm that only authorized personnel purchase restricted items and that shipment is limited only to authorized locations.
